Fingerprinting Requirements: ^{must meet ALL requirements}

You must provide current government issued photo identification, such as your driver’s license.  The information submitted on this form and on the fingerprint card must match your identification.

• Please bring government issued photo identification with you at the time of your appointment (Example: State Issued Driver’s License, Birth Certificate, Passport, etc.)
• If you have any documents that are required to be filled out by the fingerprint technician, please bring them with you.

Pricing:

Fingerprinting costs $30.00. 

Appointments for fingerprinting will be processed on Tuesdays to Thursdays between 8:00 AM to 4:00 PM.  Please note you will be asked to pay now to reserve your appointment time.
